Contact OAR to Correct Eye Doctor's Name or Address

To correct information on the "Find an Eye Doctor" database, please follow these instructions:

1.    Click Here and go to the "Find an Eye Doctor" web page.

2.    On the Find an Eye Doctor web page, enter your city or county to display your current information.

3.    Copy your current information and paste it on the below form.

4.   Below your current information, please type the correct information and include your county.

5.    Fill-in your e-mail address.

6.    Hit "Submit".

7.    Your information will be corrected within 2 business days.   Thank You
