(To qualify to participate in the Ohio Amblyope Registry, you must be an ophthalmologist or optometrist who sees children from Ohio.  Your practice may be located within Ohio or outside of Ohio in an adjacent state (i.e., Indiana, Michigan, West Virginia, Kentucky), as long as you see children from Ohio.  If you have more than one practice location, please submit this form for each practice location.  This information will be placed on the "Find an Eye Doctor" page and used for correspondence.  Upon registering, you will be sent 20 Registration Forms and a Form Holder for each location, if applicable.)

Why the Ohio Amblyope is Important to You and Your Patients

The Ohio Amblyope Registry is the first statewide registry in the United States for amblyopic children and their families. The Ohio Amblyope Registry is designed to remind your patients to keep their appointments with you and to ensure that the patients follow through on your treatment regimen. If they move into your area of Ohio, the web site page "Find An Eye Doctor" will allow the parents to easily find you in that part of the state. The goal of the Ohio Amblyope Registry is to serve the needs of Ohio’s children with amblyopia, their families and their eye doctors.

The Ohio Amblyope Registry is funded through the Save Our Sight program for children, which obtains voluntary donations through a dollar check-off box on Ohio license plate renewal forms. The Ohio Department of Health manages the Save Our Sight fund. All services provided by the Ohio Amblyope Registry are free of charge.

We would like your help and participation in making the Ohio Amblyope Registry a success. Below, under Summary of Services, is a description of the literature that will be sent to parents who register their children with the Ohio Amblyope Registry as well as the timing of the postcard reminders and phone calls, if the parents sign-up for case management.

Patients will receive those benefits as outlined under Summary of Services.
If you become one of our top 20 eye doctors who refer patients to the Ohio Amblyope registry, your practice will by be highlighted on the Top 20 Referring Eye Doctors page on the web site.

All that we ask is that you make the Ohio Amblyope Registry registration forms available to the parents of amblyopic children.  If you would like the parents to receive periodic letters to remind them that it is important to keep their appointments with you, please ask them to sign-up for the case management portion of the Registry.

 

Ohio Amblyope Registry Summary of Services

If the parents register their amblyopic child with the Ohio Amblyope Registry, they will be sent the following brochures from Prevent Blindness America (1):

Signs of Possible Eye Trouble in Children
Common Eye Problems
Amblyopia (lazy eye)
Strabismus

All literature has been reviewed and approved by the Medical Directors (one Ophthalmologist and one Optometrist) of Ohio Amblyope Registry.

2.  If you prescribe occlusion therapy, the amblyopic child will receive six (6) free boxes of eye patches as well as two (2) cloth patches for covering one side of the eye glasses.  The family also might qualify for assistance for low income families and receive up to 30 boxes of patches free of charge (click here for more details on the free eye patches program).

3.  If you prescribe occlusion therapy, the Eye Patch Club packet includes the following: Newsletter about tips and techniques for promoting compliance and professional advice from optometrists, ophthalmologists and orthoptists; A classroom guide for teachers and fellow students about lazy eye; an Eye Patch calendar with stickers, a refrigerator magnet and a pen pal form in the event that the older child wants to join a pen pal club of other amblyopic children. The Eye patch Club packets are restricted to children between the age of 3 and 9 years who are currently undergoing occlusion therapy.

 

If the Parents sign-up for case management, they will receive:

Periodic newsletters from the Ohio Amblyope Registry, including articles about amblyopia by the Medical Directors, hints for effective patching, research reports and other useful information.  The newsletters also contain information about the free patches program and information from the Ohio Department of Health.  Certain families may also receive periodic phone calls and/or e-mails about the importance of keeping their eye doctor appointments.

 

If the parents need financial assistance:

Parents are asked to talk with their eye doctor about any financial assistance that might be available. Parents can also seek financial assistance through the Combined Programs Application available from the local Department of Job and Family Services.  Call 1-800-324-8680 (voice) or 1-800-292-3572 (TDD) for further information about the Combined Programs Application. 

If the patient is undergoing patching therapy for lazy eye and if the family meets certain residency and family income requirements, the patient may qualify for free patches from the Ohio Amblyope Registry!  Click HERE to learn more about the free patches program. 

 

Information on Current Amblyopia Research in Ohio

If you are an eye doctor registered with the Ohio Amblyope Registry and would like to receive  information on amblyopia research currently being undertaken in Ohio, please submit the following form, below, and we will e-mail you the amblyopia research file.  The amblyopia research file is composed of amblyopia research notices submitted to the Ohio Amblyope Registry.  In order to be included in the amblyope research file, the research protocol must have been reviewed and approved by a human subjects Institutional Review Board (IRB).  Before research information is included in the amblyopia research file, the Principle Investigator (PI) must submit proof, in writing, that the amblyope research protocol has been reviewed and approved by their respective IRB.  Private office based amblyopia research, that has not been reviewed/approved by an IRB, will not be considered for inclusion into the amblyopia research file.

If you are currently undertaking amblyopia research and would like an e-mail notice of that research included in the amblyopia research file (and sent to eye doctors in Ohio who request such information), please submit written IRB confirmation that the study has been approved by an IRB to the Ohio Amblyope Registry (see address below submission form) AND e-mail the notice about the study that you would like included in the amblyopia research file.  Please limit such notices to no more than two pages.
