Patches To fill-out the free patches form, your child must first be registered with the Ohio Amblyope Registry. Click Here to register your amblyopic child with the Ohio Amblyope Registry. Once your child is registered with the Ohio Amblyope Registry you must complete a free patches form (Click Here for the free patches form). Once your child is registered with the Ohio Amblyope Registry and you have completed the free patches form and you have been approved to receive free patches, you can fill-out and submit the form below for a renewal of your patches. Participants are limited to a total of 30 boxes of patches, in lots of six (6) boxes. You can also call the Ohio Amblyope Registry at 1-877-808-2422 to renewal your supply of patches, but make sure you have your Approval Number available. Only fill-out and submit a patches renewal form if you have an approval number from the Ohio Amblyope Registry. The applicant will receive an approval number upon approval of their patches application form. |
